Protocol for management of Birth Asphyxia
Mild Asphyxia - APGAR scores 6-7
Moderate Birth Asphyxia -APGAR scores 4-5
Severe Birth Asphyxia-APGAR scores 1-3
1.Maintain Airways
Monitor breathing: Give Oxygen if indicated or SPO2<90%
Circulation:Monitor CRT, HR,Extremities if in shock correct with IV N/S 10mls/kg bolus
Fluids: Maintenance fluids (restrict fluids to 60mls/kg for day 1)
Control seizures
Im phenobarbitone-loading dose -15-20mg/kg stat
Maintenance dose -5mg/kg/24hrs in divided doses x 48hrs
Antibiotics(Give antibiotics only if severe birth Asphyxia or mild /moderate Birth Asphyxia with septic risk factors)
IV X-penicillin 100,000 IU/kg/dose tid
IV gentamycin term babies-5mg/kg daily/preterm baby -4mg/kg daily.
Keep NPO x24 -48 hrs(if moderate or severe birth asphyxia)
Monitor blood glucose 3 hourly
Maintain normal temperature (36.5-37.4)
Monitor HR,RR,Temp Color,CRT,SPO2 and urine output
Take sample for FBC,Renal function test/Electrolytes
